We help to ensure safety and accuracy and provide assistance with managing medication instructions from your doctors, correlating and looking for interactions, safety issues and changes in your health due to your meds. We also filled medicine boxes weekly, biweekly or monthly to ensure that medications are taken accurately. 

 We provide careful, coordinated discharge planning to ensure a smooth, safe transition from the hospital or rehabilitation center to home, assisted living, or another appropriate care setting.

Are you wondering if your loved one is ready for the next level of care? In NY state, placement in a nursing home requires a PRI and Screen (Patient Review Instrument). A Patient Review Instrument (PRI) is an assessment tool developed by the New York State Department of Health to assess selected physical, medical, and cognitive characteristics of nursing home residents, as well as to document selected services that they may receive. 

Our nurses are certified PRI Screeners and can complete this task for you, generally within 24-48 hours. We will also help you to send it off to the right place to help your loved one with placement.
